What is your raiding experience in TBC? On lanya only kara, but my mage ive done kara / gruul / maggy / SSC / TK and ZA but quit raiding on her as i got bored and fancyed a change.
Did you have any raiding experience before TBC? (remember to specify if you had another character in those raid and who he is) Yes but all on my mage again.

Do you know anyone who is currently raiding in Odyssey, who will vouch for you ? Tsang we have been friends for along time and has raided with me meny times before and in TBC.

Why are you applying to Odyssey (this is very important)? Because i would like to raid with my healer and will not get chance to with the guild my other characers are in plus id like to be raiding with tsang and enjoy meeting new people and making new friends.

Can you keep yourself supplied on consumables for all raids? Yes

Can you enchant any new gear you get within a reasonable time frame with valid enchants for your class ? Yes
